COMPLIANCE / MEDICAL CONDITIONS
- If there is any
medical condition or other information you are aware of, that could affect
someone’s participation in a recreation program or activity, please contact the
FDL-SD Community Ed & Rec Dept. to discuss accommodations.
CANCELLATIONS/INCLEMENT WEATHER - Cancellations are posted on our website, our Facebook
pages, and Hotline (920) 906-6666. The hotline is updated only if a program is
cancelled/postponed. If the School District cancels school due to weather
conditions, all FDL-SD Community Ed & Rec Dept. activities will also be
cancelled.
CLASS SIZES - Most programs have a minimum/maximum enrollment number. If we do not meet our minimum requirement, classes will be cancelled with refunds issued or you may transfer to a different class/session.
CONCUSSION, HEAD INJURY, & SUDDEN CARDIAC ARREST STATUTES - Students and parents/guardians must sign a Concussion and Sudden Cardiac Arrest agreement form once per school year (August - July) before they participate in practice or competition. This form must be returned to the school or FDL-SD Community Ed & Rec Dept. prior to participation. Forms are available at www.fdlrecdept.com.
CONFIRMATIONS - The FDL-SD Community Ed & Rec Dept. does not send confirmations. No news is good news! You will be contacted only if there is a problem with your registration or a change in the class. If you require a receipt, please enclose a self-addressed stamped envelope with your registration or stop by our office to pick one up.
DRUG, ALCOHOL, AND WEAPON USE - The use of all weapons, drugs, tobacco products, and drinking of alcoholic beverages on any premises owned by the Fond du Lac School District is prohibited by state law.
FEE POLICY - Program fees are assessed to defray the cost of program operation. All fees must be paid in full at the time of registration. Please plan to register prior to the first class as program fees are not pro-rated. Registrations/payments are not accepted at program sites unless otherwise indicated. Discounts for certain programs are not eligible online unless otherwise noted.
GIFT CERTIFICATES - Gift Certificates are available in all denominations and may be used for all Recreation Department & Aquatic Center programs. Stop by our office to purchase your gift certificate today.
GRADE/AGE RANGES - Participants registering for any class must meet the age requirement prior to or on the first day the class begins. The ages are set to ensure the safety and benefit of the participants as well as the program leaders.
INSURANCE POLICY - The FDL-SD Community Ed & Rec Department assumes no responsibility for personal injuries or loss of personal property while attending FDL-SD Community Ed & Rec Department sponsored activities or facilities.
NOTICE OF NONDISCRIMINATION - The Fond du Lac School District and the FDL-SD Community Ed & Rec Department do not discriminate against any person on the basis of sex, race, religion, national origin, ancestry, creed, pregnancy, marital or parental status, sexual orientation, or physical, mental, emotional, or learning disability or handicap in its education programs or activities. Federal law prohibits discrimination in employment on the basis of age, race, color, national origin, sex, religion, or handicap.
PHOTOGRAPHY POLICY - The FDL-SD Community Education & Recreation Department occasionally uses photos of participants for promotional purposes. By registering for the program, you have granted us permission to use your photograph for promotional purposes unless otherwise noted.
PROGRAM DEADLINES - Unless otherwise noted, all program deadlines are one week prior to the start of the program. The deadline is used to determine if the class will be run or cancelled. Registration is permitted after the deadline, provided the program maximum has not been met.
REFUND POLICY - Full refunds will be granted only for programs cancelled by the FDL-SD Community Ed & Rec Dept. Refunds for trips and some programs will be issued only if the participant's spot can be filled from the waiting list. Refunds are not issued after a program has begun.
RESIDENTS/NON-RESIDENTS - Residents: A resident is defined as one whose primary residence resides within the Fond du Lac School District boundaries. Non-Residents: A non-resident is defined as anyone who lives outside of the Fond du Lac School District boundaries. Non-residents may be subject to an additional non-resident fee.
RETURNED CHECK POLICY - A $25 service charge is assessed on all checks returned due to insufficient funds.
SUPERVISION/PICKUP NOTICE - The FDL-SD Community Ed & Rec Dept instructors & staff are not responsible for supervision of youth program participants after a program is finished. For the safety of your child(ren), please make arrangements for prompt pickup from all FDL-SD Community Ed & Rec Dept. sponsored activities, lessons, or programs.
TYPO, HUMAN ERROR, WE GOOFED!!! - Occasionally there may be an error in days, times, registration requirements, or fees in the activity guide. When such errors occur, our staff will do everything possible to correct the situation promptly. We thank you for your patience and understanding when these situations arise.
WAITING LIST - If a class has reached its maximum capacity, you may place your name on the waiting list. If feasible, all attempts to create additional spots and/or classes will be made. There are no guarantees for placement and the waiting list is compiled on a first come, first serve basis.
